Scheduled Weekly Hours:
8-hour day shifts, variable days (United States of America)
The Care Team Coordinator functions as an important member of the organization's healthcare team and is responsible for delegated tasks under the direct supervision of a provider. The Care Team Coordinator rooms the patient, completes team documentation and preforms in between visit functions within the specific team care structure to provide excellent patient care to those we serve.
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Required Education: Graduate of a Medical Assistant program or Documented successful completion of the following courses (of their recognized course equivalent):
Anatomy & Physiology or Body Structure & Function or Human Body in Health & Disease
Preferred Education : Licensed Practical Nurse license awarded by the State of Wisconsin
Minimum Experience Required: None
Preferred/Optional Experience : Experience in healthcare
The following licensure(s), certification(s), registration(s), etc., are required for this position, which must be maintained in good standing:
Minimum Required: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
