This position will work as a Patient Care Manager who will be responsible for the non-medical case management activities.
In partnership with the Medical Care Manager this position will coordinate support and social service needs and collaborate with other health care team members to identify the patients' goals to formulate the care plan based upon assessment data and provision of care priorities.
Modifies the plan of care based on the reassessment findings and specific care needs and changes in the medical treatment plan.
Through the delivery of brief interventions the Patient Care Manager will assist patients towards behavior changes that enhance HIV treatment adherence and reduction of risk behaviors.
The Patient Care Manager coordinates behavioral interventions and support services with the patient's medical care to promote improved health outcomes.
The position is responsible for delivering patient-centered activities that focus on the patient's psychosocial needs particularly as they relate to mental health and substance abuse issues.
Three (3) years of experience working with HIV-positive patients and a working history within the last five (5) years, or educational background, that includes knowledge of STD's, HIV, Hepatitis, and Tuberculosis.
A Master's degree in one of the following disciplines: Social Work, Counseling, Psychology, and/or Marriage and Family Counseling.
Possess basic computer skills, proficiency in Microsoft Office software, data entry, and use of internet and internet research.
Have strong quality management skills; ensure compliance with all contract guidelines and regulations.
Strong oral and written communication skills, ability to build rapport with clients (i.e. customer service skills, outreach) and talk openly sexual risk taking behaviors.
Knowledge of substance abuse and mental health issues and treatment, and related sexual risks.
Knowledge and experience working with behavioral risk groups, including Injection Drug Users, Lesbian, Gay, Bi-sexual, and Transgendered Community.
Good organizational skills and ability to maintain client files, charts and test results.
