Dignity Health DCIC Program Manager in LONG BEACH, California

Responsibilities

Under the direction of the Vice President of Business Development and the Director of Community Health, the Manager, Integrated Care Center for Seniors (CICS) is responsible for the development and operation of the Integrated Care Center for Seniors. The responsibilities include program design, identification and collaboration with community partners, and integration into existing hospital programs and services. Once the CICS is established, the Manager will oversee the program services and programs including integration between the CICS TCC Primary and Specialty Medical Clinic for Seniors and the St. Mary Bazzeni Wellness Center.

Qualifications

Requirements:

  • Experience integrating services across acute and non-acute settings.

  • Minimum of two years experience in an acute care setting.

  • BS in Health Sciences, Public Health or related field.

  • Development and maintaining community partners as related to the project and the CICS future development. Knowledge of the internal workings of a health care system.

  • Knowledge of the needs of the elderly, their families and/or care givers. Knowledge of health promotion and integration of social services.
