Trinity Health Certified Nurse Assistant - Mt. St. Vincent in Holyoke, Massachusetts

The certified nursing assistant works under the direction of a licensed nurse and is responsible for the delivery of personal care that promotes the resident’s ability to function at the highest possible level. The role shall respect each resident’s rights and recognizes the individuality of each resident. All care shall be delivered at a level that meets or exceeds the resident’s expectations.

Demonstrates, promotes and integrates the Sisters of Providence Care Centers Mission and core values by:

Listen to report, obtain assignments and the condition of the Residents at the beginning of shift. This will provide information needed to understand and implement the current plans of care for each Resident.

Reviewing Resident Care Plans (and care cards) and providing for care of Residents accordingly by assisting with ADL’s as needed. Personal care may include shower/ bath, shampoo, perineal care, oral hygiene, positioning, nails short and clean, dressing, range of motion, ambulation and insuring adaptive devices are worn (i.e., dentures, hearing aids, glasses, etc.). All personal alarms should be functioning properly and in place.

Observe Residents and report any reactions or changes in condition to appropriate personnel.

Escort Residents to and from the Dining Room for meals, collect trays, feed Residents as assigned, record intake of solids and fluids and supervises those Residents who need assistance, distribution of nourishments to Residents as assigned, and taking responsibility to report to Nursing if refused by the Resident (ex., diabetic Residents).

Assist in transporting residents to and from activities programs.

Record accurately on C.N.A. Flow Sheets, Bowel and weight, intake and output sheets and reporting any significant changes to the Charge Nurse.

Responds to call bells in a timely manner and assists Residents as able or seeks assistance to meet the Residents needs.

Follows all safety, security, Infection Control (including Universal Precautions) and hazardous materials policies and procedures. Performs all tasks to assure resident and personal safety and the protection of co-workers and facility property.

Promote and participate in educational programs. Annually attends all required educational programs according to administrative policies and procedures. Maintains required certification.

Ensure that resident equipment, such as wheelchairs and lifts are clean and in good working order.


The general knowledge and comprehension skills normally acquired by obtaining a high school diploma or GED in order to complete in-service and daily documentation requirements. Graduate of an approved Certified Nursing Assistant Training Program with current certification

Ability to read, write and converse in English. A second language is helpful.

Interpersonal skills necessary to relate to persons of varying age, culture, religion, and/or ability.

Ability to remain physically active for the duration of the assigned/scheduled shift. Will bend, stoop, lift, stand, walk, and carry supplies in the completion of the assignment. In emergency situations may be required to assist in lifting a person who has fallen.


Requires prolonged standing, walking, or other constant physical movements on a regular basis and frequent lifting of moderately heavy items up to 40 pounds.

Requires exposure to blood and various body fluids, which requires caution and use of established safety guidelines and equipment.

Work requires ability to communicate orally and in writing.

Work requires some irregularity in scheduled hours because of overtime requirements, including weekend rotation, or occasional stress due to periodic cyclical workload pressures.
