Trinity Health LIFE Van Driver in Feasterville, Pennsylvania

Job Purpose: To transport and/or assist with transporting LIFE participants to and from the LIFE Center and to other appointments. To provide a comfortable, safe, and efficient transportation experience. To communicate with the interdisciplinary team, providing relevant information regarding status of participant and family caregivers, and any problems or issues. Major Duties and Responsibilities: 1. Transport participants in a safe manner in compliance with the Pennsylvania Department of Motor Vehicle licensing regulations. 2. Drive LIFE participants and potential participants to the LIFE Center from their homes and home again. Assist participants in and out of their homes, on and off the LIFE bus. 3. Drive LIFE participants to medical offices, to their homes from hospitals, and other special trips. 4. Provide receptionist with information needed to make contacts with medical offices, participants, family members, and others regarding transport scheduling. 5. Contributes to participant and caregiver satisfaction by providing high quality services and communicating effectively with participants and families. Reports to supervisor when participants/families have concerns or questions regarding services. Reports any changes observed in the condition or behavior of the participant and any unusual incidents in the home. 6. Keep vehicles clean and sanitary. 7. Regularly communicate transportation information as a part of the interdisciplinary team. 8. Provide information to the receptionist regarding any participants who will not be attending the center and the reason. 9. Transport/assist with transport of supplies, specimens, pharmaceuticals, meals, and mail. Helps receive and store food and other deliveries at center. 10. Other duties as assigned. Minimum Qualifications: Education: High School Diploma or GED preferred. Experience: Transportation and human services experience. Experience driving large buses or vans. Commercial Driving License, Good driving record. CPR Minimum of one year experience in working with people with disabilities or elderly population Possess: Ability to communicate well in English, orally and in writing. Ability to interpret and follow oral and written instructions.
