LIFE St. Mary Title: Home Care Lead CNA and Scheduler Department: LIFE (Living Independently For Elders) Reports To: Home Care Supervisor Position: Coordinator/Lead Reviewed/Revised Date: 1/20/2017 Exemption Status: Non-Exempt Job Purpose: The purpose of this position is to assist the Nurse Manager:

  • The completion of the weekly schedules

  • To assist with the training of new aides

  • Represents the personal care staff on the Interdisciplinary Team

  • To assist with supervisory visits

  • To represent the CNA at all assessment and re-assessments.

  • Continues to evaluate and plan needs of CNA staff for further education.

  • Performs personal care services as needed.

  • Supervises Home Care Department Major Duties and Responsibilities: 1. Prepares weekly day home care schedule for coverage of personal care, companion, and respite services. Determines best match of staff with participants for highest quality of care. Adjusts schedule daily as needed to cover unscheduled absences, special events and other changes. 2. Coordinates aide schedules with the Home Care Nurse Manager as needed. 3. Schedules and tracks review of aides’ competencies by LIFE. 4. Documents all care provided by personal care assistants and homemakers for DataPACE2 in accordance with required timeframes. 5. Makes home visits with Nurse Manager to create personal care plans with participants and families, and to follow-up. 6. Assists with the orientation and training of new home care personal care assistants. Contributes input to performance evaluation of home care personal care aides prepared by Home Care Manager. 7. Performs personal care assistance for LIFE participants as scheduled and on a fill-in basis as necessary. 8. Assist manager with supervisor visits. 9. Tracks credentials of all working CNAs. 10. Reviews care plans with RN Manager to insure their accuracy. 11. Provides flow sheets to agency for placement in participant's home. 12. Assists with review of flow sheets. 13. Other duties as assigned. Minimum Qualifications: Education: High School graduate or GED equivalent. Current CPR certification or willing to be trained within 6 months. Experience: Two years experience as a Personal Care Assistant or CNA, and/or experience as a scheduler. Basic clerical knowledge and experience. One year of experience with a frail or elderly population. Possess: 1. Ability to communicate well verbally. 2. Excellent personal care skills. 3. Positive interpersonal relations with fellow employees, participants in the LIFE program. 4. Punctuality, dependability, and flexibility. 5. Maturity and ability to handle conflict. Age Specific Criteria: Demonstrates skills and knowledge necessary to assure assessment, treatment or care is appropriate to the age of the patients served. Working Relationships: Interacts regularly with other hospital departments, providers, LIFE participants, family members, and LIFE staff within and outside the organization on a daily basis. Contacts within the organization involve all levels of individuals and in various situations that include one on one contact, meetings, and committees. No special requirements exist to communicate in languages other than English. Environmental Conditions: Working conditions are indoors primarily in an office at the adult day center and also at participants’ home in the community. Normal health and accident hazards found in a health care setting and when visiting homes in the community are encountered on a daily basis. Exposure to blood, body fluids, and other infectious hazards. Equipment Used: Devices associated with an adult day center, home care, and business office work environment can be used on a daily basis to include electrical and non-electrical equipment. The Home Health Aide (HHA) performs varied participant care activities and related support services necessary in caring for the personal and comfort needs of participants. The HHA works in participant homes and may also work in the LIFE center. In performing all duties, HHA complies with LIFE policies and procedures


Valid Drivers License

Certified Nurse’s Aide (CNA) with current certification, or Certified or Registered Medical Assistant, or a trained home health aide meeting the competencies in PA Code, Title 28, Part IV Subpart H, Chapter 611.55
