Family Service Association - Fall River Case Manager (per diem) in Fall River, Massachusetts

We have an exciting opportunity for a per diem Case Manager in our Guardianship Program. The Case Manager will be responsible for all case management duties including but not limited to: doctor consultation, appointments, collateral contacts, hospital admissions and discharges, nursing home placements, medical certificates, treatment plans, funeral arrangements and monitoring of personal needs. The Case Manager will have regular visits with assigned clients at their place of residence within the state of Massachusetts. Must be familiar with the health insurance for each assigned client.

The ideal candidate will be a graduate of an accredited university with a BS in Psychology, Social Work, Sociology or related field and prior experience working with disabled/elderly adults in a community and/or long term care setting; ability to work with family members of disabled/elderly individuals, knowledge of community services for elders and disabled individuals helpful; must have the ability to make sound judgment decisions about medical care, placement and personal needs; reliable transportation is also required.

The hours will be flexible; Tuesday, Thursday and Friday.

The Guardianship Program at Family Service Association integrates the legal guidelines regarding guardianship of an incapacitate person set forth by the Massachusetts General Laws with a quality clinical case management approach and a full complement of fiscal fiduciary services. The Program offers a variety of specialized services to those needing assistance in managing their personal, medical, financial and legal decisions.
