OBJECTIVE: Develops, coordinates, and directs program activities for farmers/ranchers and/or their family members and/or individuals employed in the agriculture arena with intellectual, physical and/or developmental disabilities by performing the following duties personally or through subordina KEY RESPONSIBILITY AREAS: * Manages a caseload of farmers/ranchers and/or their family members and/or individuals employed in the agriculture arena consisting of all disability areas and coordinates the utilization of multiple health-care related or agricultural related community agencies. * Conducts assessments on participants involving the use of specialized knowledge and applications associated with social work, rehabilitation, psychology, or occupational therapy. * Interviews farmer/rancher, their families, and other responsible individuals; assists in completing the application for services; collects basic data and obtains appropriate additional information from other agencies. * Prepares or dictates case progress notes, after-site visit letters, site-visit narratives, and other reports concerning the participant's case. * Develops, in cooperation with other staff, a plan of care for each individual participating in the program. * Offers solutions to participants to mitigate or remove physical, environmental and societal barriers that prevent participants from maintaining their independence and self-sufficiency. Provides resources for Behavioral Health when needed or requested. * Coordinates inter-disciplinary team meetings to discuss cases concerning eligibility, support plans, progress, and possible changes in the service program. * Monitors services to ensure that the terms of the authorization are being fulfilled by the vendor, to check on the quality of services, and to review participants progress. * Identifies and develops relationships with community-based and government agencies that will be able to provide additional resources or aid in the purchase of adaptive technology for participants. * Visits service agencies, attends meetings, and confers with subordinates to obtain additional information concerning community resources for persons with disabilities. * Submits Rehabilitation Technology evaluations to the Colorado Division of Vocational Rehabilitation on an as-needed basis. * Trains subordinates by providing information concerning case management practices, administrative rules and regulations, community resources, vendors, and programs. * Develops and delivers educational workshops on topics relevant to farmers/ranchers, their families, outside professionals and other interested parties. * Markets the program to healthcare providers, agricultural producer organizations and individuals to provide education and increase program enrollment. * Assists ETO Director and GOC's Development Department in writing grant proposals that benefit the AgrAbility program. * Meets or exceeds revenue goals while minimizing program expenses. * Ensures that all program goals are met and affects changes as required. Actively participates in and contributes to the annual short-term and long term strategic planning process. * Submits month y, quarterly and annual reports as required by GOC, Colorado State University and the National AgrAbility Project (NAP). Manages and is accountable for program data reporting including the design and coordination of data systems and procedures. Provides data outcomes for reporting of program performance through dashboards, Annual Statistical Reports (ASRs), etc., executive team, board members, stakeholders and partners. * Prepares and maintains accurate expenditure records. * Exercises independent judgment in making decisions related to participant services; receives general supervision from the Director of Career Development Services. * Oversees the management of the day-to-day operations of the AgrAbility Project. * Performs any other duties as required or assigned. EDUCATION AND EXPERIENCE Bachelor's degree (B.A.) from four-year college or university and a minimum of three to five years related experience and/or training. Master's degree (M.A) is preferred. Background and proven experience in human services, serving the disabled population, and/or occupational therapy is RESPONSIBILITY FOR FUNDS, PROPERTY and EQUIPMENT Employee may be driving a Goodwill vehicle with consumers to events and work locations and is responsible for adhering to the organization's safety and liability requirements.
