Trinity Health Manager, Ambulatory Care Coordination in Conshohocken, Pennsylvania
*General Responsibilities *
Manages daily operations and provides oversight for RN ACO Care Coordinators, Ambulatory Population Health RNs , Community Health Workers and Social Workers to ensure safe, effective transitions of care. Coordinates daily department operations and supports ongoing problem resolution. Establishes tools, processes and systems to optimize department effectiveness. Serves as Liaison with Payers/ACO/MPN. Monitors the post acute partnership program. Project Management responsibility for Care Coordination projects. Develops and maintains Ambulatory Quality Improvement program. Oversees new RN/SW Orientation.
This position is based out of Conshohocken, PA.
Education and Training: RN with MSN or other healthcare related graduate degree required or in progress, or MSW. Demonstrated quality, pay-for-performance, performance improvement, and/or care coordination experience required.
Certification and Licensure: RN or MSW

General computer knowledge and capability to use computers and EHR required.
Working environment requires an ability to meet deadlines with frequent interruptions, ability to work under pressure and manage changing priorities.
Some travel required, including onsite review at hospitals, ancillary facilities or physician’s offices. Additional time commitment may be required to complete work responsibilities.
Must be able to set and organize own work priorities, and adapt to them as they change frequently
