Our Social Worker LISW provides medical social work services as ordered/referred by the attending physician to Hospice
Patients in their home, according to Hospice policies and procedures and state and federal regulations governing Hospice care. Uses professional judgment for initial and ongoing assessment, planning, and evaluation of psychosocial, economic, and environmental needs of patients and families. Acts as a consultant to Hospice staff for medical social work service needs.
- Develops appropriate care plans and establishes short and long-term goals; measures progress or lack of progress in achieving these goals as part of the overall interdisciplinary
care planning process.
Plans for coordinated Hospice care with other disciplines or community resources; consults with others as necessary.
Observes, evaluates and reports to Hospice Interdisciplinary Team re: patient/family psychosocial status, and significant changes for patients and family's coping status.
Assesses safety factors in the patient's residence regarding conduciveness to provision of care, including but not limited to safety, accessibility of bathroom, availability of telephone and adequacy of in-home caregivers.
Responds to referrals for social work within 48 hours of receipt of referral.
Participates in admission process providing psychosocial assessment of patient and family. Assists in identifying ""Red Flag"" families and communicates to IDT. Assists in resolving problems of increasing dependency or social isolation through counseling, group meetings, and referrals. Assists in resolving of patient/family attitude toward Hospice services in area of dissatisfaction. Assists in obtaining medical, dental, psychiatric, or rehabilitation services.
Master's degree from school of social work. Graduated from a program accredited by CSWE (Council on Social Work Education).
Licensed Social worker by the State of Ohio Counselor and Social Worker Board (LSW or LISW)
Minimum experience preferred is two (2) years in social work; one (1) year in a medical setting.
Current knowledge of county, state, and federal programs (Medicaid, Medicare, Social Security, SSI and Veterans benefits) in particular, and Medicare Hospice Benefits.
Ability to counsel terminally ill patients and families in social service areas as well as psychosocial areas.
Ability to operate a motor vehicle; possess a valid driver's license and have access to a functional automobile during work hours.
Criminal fingerprinting completed as required by Ohio law
