TeamMD is a new model of care focused transforming care for high-risk Medicare and Medicaid patients in their home. We are currently expanding our team across Tennessee, and we are looking for Nurse Practitioners who bring compassion and passion, energy and focus to their work every day.
In this role, you will complete In - home assessments of chronically ill and high risk patients in a variety of environments including private homes, assisted living facilities, nursing homes, skilled nursing facilities, and / or hospital settings. You will also provide consultation for post-acute stabilization as well as temporary support for patients transitioning from higher levels of care. You will collaborate with a multidisciplinary team to engage resources and strategies to address medical, behavioral, functional, and social barriers to care.

Primary Responsibilities:
Complete comprehensive assessments in a variety of settings, to address gaps in care and facilitate complete and accurate coding of complex health conditions
Review, interpret, and integrate data to guide best practices in diagnostic and therapeutic procedures
Provide temporary stabilization services for post-acute and transitioning patients
Manage medical emergencies, triage phone calls, prescribe medications, and make referrals for ongoing services
Complete full and appropriate documentation to support professional billing for services
Facilitate clarification of patient and family goals of care to support development of a comprehensive care plan
Provide education and information on health conditions, illness trajectory, and treatment options to patients, families, caregivers, and providers
Provide input to organizational quality initiatives and staff education
Participate in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of service delivery

Ability to work with diverse care teams in a variety of settings
Ability to work independently
Required Qualifications:
Graduate of an accredited Master's Degree in Nursing (MSN) program
Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
Current unrestricted nursing license in good standing in Tennessee
Current DEA licensure / Prescriptive Authority in state of Tennessee
2+ years of post - graduate experience working as a Nurse Practitioner in an Adult, Family, Internal Medicine, Geriatric, Palliative or Hospice setting
Advanced knowledge of and experience with symptom management
Computer literate and experience with Electronic Medical Records
Must have access to high speed internet for home office set up
Willingness to travel up to 50 to 75% for face to face meetings throughout the state of Tennessee. *Financial assistance for relocation will not be provided.
Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
Preferred Qualifications:
Experience in Gerontology or Internal Medicine
RN / NP experience working in home health or home care setting
Experience working in a Psychiatric or Mental Health setting
Excellent administrative, organizational and verbal skills
Experience with effective communication skills with the geriatric or Medicare population
