Trinity Health RN, Registered Nurse - Outpatient Behavioral Health Clinic in Chelsea, Michigan

General Summary: The Program RN will participate as a member of a multi-disciplinary team to provide services to patients in the department’s Partial Hospitalization Program. The RN will conduct Nursing Assessments and provide education, both individual and in groups, to program patients.

The Assessment Services Clinician (RN) is responsible for determining the appropriate placement of incoming referrals within the continuum of behavioral health services. Generally, he/she conducts a telephone intake, assessing the patient’s presenting problem, current symptoms, and acuity and makes a determination as to the patient’s appropriateness for admission to the hospital’s behavioral health inpatient, intensive outpatient or outpatient treatment programs. The Assessment Services Clinician (RN) may also conduct face to face patient assessments at the patient's bedside (in the case of an individual hospitalized in a SJMC medical bed) or in the emergency department at the request of the attending physician, or as a walk-in in the outpatient clinic.

RN’s in Assessment roles will assist patients with questions, concerns and needs related to medications, appropriately consulting with the on-call physician in the absence of the attending psychiatrist.

RN’s in Assessment roles will also assist patients with prescription refills where the attending psychiatrist has given prior authorization to do so. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participates in their resolution.


Discusses patient and hospital information only among appropriate personnel in appropriately private places.

Maintains knowledge to perform the duties of the job.

Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.

Provides quality patient care by considering the age specific, development and cultural needs through competent clinical practice.

Demonstrates unit/area competencies Identifies and assists in resolution of problems related to safety, sanitation and maintenance of the environment.

Practices effective communication in all interactions (patient, visitor, co-worker, etc.) Promotes a positive work environment by demonstrating teamwork.

Required Education, Experience and Certification/Licensure

Must have an associates or bachelors of science degree in Nursing from an accredited school of nursing, and current Michigan state licensure as a registered nurse. A minimum of three years psychiatric nursing experience is required. Experience working in a multi-disciplinary team approach is highly desirable. Experience with dually diagnosed population preferred.
