Trinity Health Patient Companion in Chelsea, Michigan
Under the direction of the registered nurse the patient companion provides continuous observation and surveillance of an assigned patient and will notify the nursing staff if the patient requires assistance, exhibits changes in behavior or requires other nursing intervention. The patient companion may perform specific activities as assigned by the patient’s primary registered nurse.
Patient Companions will need to have flexibility in their coverage schedule. We are looking to have patient companions work 4 to 8 hour shifts Monday - Sunday. This schedule is a flexible/contingent schedule with an on call component.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Receives shift change report from RN and previous patient companion regarding patients’ anticipated behavior and safety risks. Throughout the shift, completes patient monitoring log. Provides hand-off information to next shift patient companion and RN. Introduces self to patients/families and explains that they are there to assist in maintaining a safe environment. Remains with, and observes the assigned patient at all times. Patient must be in direct observation. Arranges meal and break time with the patient’s nurse so that patient is never unattended. Performs specific patient care activities as assigned by the patient’s RN considering age specific, developmental, cultural and spiritual needs.
- Setting up food tray, assist with eating
- Assisting patient with basic hygiene needs (bathing, brushing teeth)
- Offering fluids
- Accompanying patient on walks
- Assisting patient to bathroom or chair
- Providing Diversionary activities
- Replacing nasal cannula
- Re-applying intermittent compression devises (ICDs or SCDs)
Immediately notifies nursing staff if the patient requires additional assistance, exhibits changes in behavior, or requires other nursing interventions. Maintains the confidentiality of information acquired pertaining to patient, physicians, associates and visitors to St. Joseph’s Mercy Health System. Discusses patient and hospital information only among appropriate personnel in private places. Follows all HIPAA policies and procedures. Behaves in accordance with the Mission, Vision and Values of Saint Joseph Mercy Health System. Demonstrates all SJMHS “Performance Standards: Focus on Others*”. *Fully integrates Relationship Centered Care into role. Follows all SJMHS policies and procedures including infection control (use of body substance isolation and use of personal protective equipment when in contact with body fluids). Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers. Reports all preventable hazards and unsafe practices immediately including near misses and actual errors either to management or using the SJMHS anonymous reporting system (VOICE).
REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE
Education: High school diploma
Experience: Able to maintain professionalism and self-control in difficult or stressful conditions.
REQUIRED SKILLS AND ABILITIES
Interpersonal skills necessary to effectively communicate with patients, families, and other employees. Ability to:
- Follow instructions and record patient information.
- Read and write in order to follow instructions and record patient information.
- Sit for long periods of time and remain alert without distraction from personal business or other diversions.
- Maintain professionalism and self-control in difficult or stressful conditions.
- Concentrate and pay close attention to details for over 90% of the time.
- Must be able to lift 15-30 lbs.
Able to work in area with potential exposure to infectious agents and/or contaminants.
