University of Virginia LPN - Grand-Aides – Emergency Medicine in Charlottesville, Virginia
This position to be supported for 2 to 3 years pending project funding.
This unique LPN position with our Grand Aides Program performs patient visits in the home and community setting to observe, record and report patient responses to protocol questions to the supervising practitioner. This position will report to and work directly with nurse practitioners in the Grand Aides Program to assist in the care patients discharged from the hospital and Emergency Department. Successful applicants will have high self-accountability, organization skills, and willingness to travel and work independently in this highly autonomous role.
Under the direction and supervision of the nurse supervisor, administers protocols, records patient responses and reports findings to supervising nurse.
Perform patient visits in the home and community setting for the purpose of protocol administration, patient teaching/reinforcement, and communicating patient condition back to nurse supervisor via telemedicine (skype, facetime, etc).
Competence and confidence in the use of technology, including iPad.
Communicates patient care information to the patient and other health care team members, by verbal report and/or documentation of patient responses in the electronic medical record according to facility policy and procedure.
Takes and records vital signs (blood-pressure, pulse, temperature, respiration, weight and pulse oximetry).
Contributes to the development of the patient plan of care under direction of a nurse practitioner.
Uses Infection control measures to prevent the spread of the infection in the home, between family members and for self-protection.
Respects the rights, independence, privacy and confidentiality of the patient.
Provides mental and emotional support to patient and families.
Maintains a cooperative, positive relationship with staff of the healthcare facility to facilitate optimum care for the patients.
In addition to the above job responsibilities, other duties may be assigned.
Education: High school diploma or equivalent.
Experience: 1 year medical experience
Licensure: Licensed to practice as a Practical Nurse (LPN) in the Commonwealth of Virginia. Possess an unrestricted Driver’s License. Not more than 1 moving violation in last 12 months. Current American Heart Association CPR.
Job requires sitting/standing for prolonged periods, frequently bending/stooping/walking (distance: 1 – 2 miles/day), climbing (ladder, steps) and reaching (overhead, extensive, repetitive). Ability to lift/push/pull 20 – 50lbs.
This position may be exposed to electric current while working on electronic equipment, exposed to extreme temperatures in steam tunnels and required to stand on different surfaces.
