Leake and Watts Services Reunification Specialist in Brooklyn, New York
As part of the Enhanced Discharge Supports for Stronger Family Permanency initiative, the Reunification Specialist (RS) will work with families with a Permanency Goal of Return to Parent to strengthen the reunification process and connect families to community services and supports. The Reunification Specialist will report to the Supervisor/Clinical Director and work collaboratively with case planning and other Permanency Support Services staff.
Receive and track referrals from case planning team of families that are on track to trial discharge and need support and connections for reunification
Perform regular assessments of parent behavior change
Make appropriate referrals for families to support the reunification process
Facilitate Evidence-Based Model workshop: Parenting Journey
Mentor families by modeling appropriate interpersonal and parenting behavior along with skills of daily living
Facilitate Family Time coaching techniques during family visitations
Plan and host events for families whose children are on trial discharge (i.e. Family Fun Night)
Establish and maintain professional relationships with pre-existing providers already involved in families’ lives
Establish and foster relationships among local schools and other community providers to support the families
Conduct field visits to families’ residence/schools/service providers
Maintain accurate records of all interventions and involvement with families by documenting contacts within 72 hours
Responsible for attending management meetings and providing updates on youth services to the foster care management staff
Contribute to unit’s overall efforts to enhance permanency options for families by performing tasks in a timely manner; providing updates on a regular basis; observing the agency’s work schedule and attendance policies; and follow all agency regulations as outlined in the Employee Manual
Execute all other tasks as directed by supervisor/Clinical Director to support the families being served
Complete all other tasks as assigned by Supervisor
- Master degree in Social Work or related field. LMSW preferred.
- One Year working directly with Children, youth and their families in a Foster Care or Child Welfare setting.
