What You Will Do
As an Associate with Seton Family of Hospitals-TX, you will have the opportunity to investigate, assessand planinterventionto help patients cope with social, emotional, economic and environmental problem.
Interviewpatient, assesspriorities and documentcase activity.
Consultwith team manager, statutory and voluntary agencies and patient's relativeto ensure compliance with patient's treatment plan.
Assistclient/familywith obtaining community assistance by referral to proper resource.
Developand maintainworking relationshipwith community health, welfare and social agencies and seekcreative means to assist the client with his/her needs.
Supportmanagement of concurrent review, discharge planning processes, social services, home care and other post discharge need.
What You Will Need
Social Workercredentialed from theTexas State Board of Social Worker Examinersobtained prior to hire date or job transfer date.
High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required
- 3 years of experience preferred.
