JOB REQUIREMENTS: Social Worker Mental Health and Substance Use Ashland County Health & Human Services is accepting applications for a full-time Adult Services Social Worker supporting Mental Health and Substance Use clients. This position will develop and implement individualized treatment plans and provide case management for persons who are involuntarily committed, have chronic mental health conditions and/or have a substance use disorder. Job Summary: 1) Provide services to Mental Health and SUD clients; which include assessment, program eligibility determination, coordination, information and referral, advocacy and Targeted Case Management. 2) Determine the range of services needed through interviews, home visits, collateral contacts, etc. 3) Prepare case related documents including individual services plans, reviews, forms, correspondence, HSRS/PPS reporting, contracts, court reports and case record documentation. 4) Coordinate services with other agencies/disciplines/community members such as medical personnel, therapists, nursing homes, Social Security Administration, family members, guardians, etc. 5) Authorize expenditures of funds (with supervisor approval) within identified service plans. Monitor expenditure of funds for effectiveness and efficiency, and to ensure against over-expenditure. 6) Participate in unit and department meetings, work-related committees and trainings. 7) Coordinate services with CSP/CCS. 8) Attend Adult Drug Court as Ashland County H&HSD representative. 9) Provide informational presentations to other staff, professionals and community groups related to department programs. 10) Participate in supervision/consultation. 11) Share in the orientation and training of new workers. 12) Back-up Adult Protective Services Social Worker as needed. 13) Coordinate the Chequamegon Coalition on Emerging Drugs 14) Perform other duties as assigned. ***** OTHER EXPERIENCE AND QUALIFICATIONS: Qualifications: 1) Four-year degree from an accredited college in social work, sociology, psychology, criminal justice, or other approved human services program. 2) Minimum one-year experience providing community based direct services to individuals with mental health or alcohol related problems. 3) Valid Wisconsin Driver\'s License and reliable transportation and/or the availability to have transportation services available as needed. ***** APPLICATION INSTRUCTIONS: Other: For an application packet and full listing of job description and qualifications, contact the receptionist at 630 Sanborn Avenue, Ashland, WI 54806.
