Trinity Health Registered Nurse, Contingent - Weight Loss Program in Ann Arbor, Michigan
- POSITION SUMMARY:
Establishes, implements and monitors a plan of care for the medically supervised patient in accordance with the HMR weight management program. Works closely with the HMR Team, including clinic staff, Clinic Coordinator, NP/PA and MD, to provide exceptional medically-related interventions, options and education for the individualized clinic patient. Weight loss clinics are scheduled in the evening along with group classes; position requires 1-2 evening shifts per week until 7:30 p.m., or later.
Assists with all aspects of the HMR clinic function including stocking and maintaining clinic equipment and rooms; maintaining clinic forms and files; tracking clinic related invoices; inducting patients; tracking, documenting, and communicating medical fees to patients and program coordinator; communicating with patient PCPs regarding patient participation and progress in the program.
Participates in the hiring of new program staff.
Trains, schedules, and supervises clinic staff, providing vacation coverage when necessary.
Communicates with program staff at weekly patient care meetings and as needed to provide a continuum of care for patients.
Collaborates with program staff to facilitate the patient enrollment process by answering inquiries, orienting potential patients and scheduling induction appointments.
Provides group health education as if relates to medical weight management or health and medical risk factors.
Conducts periodic program evaluations, including the yearly Health Risk Appraisal project, and disseminate results.
Identifies appropriate health risk appraisals and methodologies to incorporate into clinic services.
Maintains knowledge of current trends in the field of obesity treatment.
Participates in hotel based and phone trainings and communicating best practices and new interventions with program staff.
Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
Maintains the confidentiality of information acquired pertaining to patient, physicians, employees and visitors to St. Joseph Mercy Hospital. Discusses patient and hospital information only among appropriate personnel in appropriately private places.

EDUCATION, EXPERIENCE AND OTHER REQUIREMENTS:
Requires graduation from an accredited program and current Michigan licensure as a Registered Nurse or Nurse Practitioner.
Two to three years related experience. Weight Program experience Preferred.
Ideal candidates would identify their own opportunities for professional growth, have participated in QI and evidence-based practice activities, integrating changes into practice and identify concerns, initiating appropriate follow-up.
