Anticoagulation Clinic Pharmacist

*Role: Clinical Pharmacist – Anticoagulation Management Schedule*: Monday – Friday

A clinical pharmacist within the Anticoagulation Clinic, ACC, will be responsible for Collaborative Drug Therapy Management of physician referred patients. The key responsibilities of a pharmacist practicing under a CDTM program will include a complete patient review and documentation and working closely with both the patient and the referring physician to promote optimal anticoagulation therapy outcomes.

Primary Duties and Responsibilities

  • Provide safe, effective, and economical care to patients
  • Provide patients consistent management, education and follow-up care through data evaluation, ordering and interpreting laboratory tests, and patient communication
  • Maintain proper patient care documentation
  • Ensure continuity of care by identifying non-compliant patients, and at risk patients for adverse reactions and making an intervention
  • Act as an anticoagulation expert resource for St. Peter’s Health Partners


  • BS in Pharmacy, or Pharm.D. with active NYS license and registration
  • At least 3 years of licensure with BS or 2 years with MS or Pharm.D.
  • At least 1 year of clinical experience
  • Either board certification or completion of a residency program required
  • Strong collaborative, interpersonal and organizational skills
  • Strong computer skills working with electronic health record
  • Experience working under a CDTM desirable
  • Clinical anticoagulation experience desirable
  • APPE student and Resident precepting experience desirable
